Unable to add Hemoglobin or BMI to custom form

When adding the Hemoglobin lab or BMI to a custom form it does not save in the Hemoglobin group box or the vital signs group box. A user is able to add the Hemoglobin lab to a new form using build 554. In testing we were able to add to a new form with these fields in build 580, but not to an existing form. This happened inconsistently.
