Provider Name and Primary Provider not working in custom reports

Provider Name was removed from CAREWare but somehow was not removed from the custom report setup. Reports will have cw_table/domain errors when running it in Central Administration. A work around is to remove the field and add in the provider that is appropriate to the the report type. ie. lab report = test provider.

Primary provider causes an error when added to a lab report. Error states that lab_dmn is not joined. Unsure if this will occur with other report types.
