Custom Field on Custom Report appears twice

When you add fields to a custom report field selection, and the same field appeared twice, like this:
Case Manager (Annual Custom) Case Manager (Annual Custom)
If a user uses one of those fields, they get the current case manager, and if they use the other, they get an old case manager.
I checked the custom field setup screen for that domain, and the field is only defined once. Is this maybe a Case Manager field from another domain?
