Cannot add new service via Rapid Service Entry

Can't add new service with rapid entry. All permission granted to user and domain. Error generated is:

   at Client.rapidServiceEntryList.lnkNewService_LinkClicked(Object sender, LinkLabelLinkClickedEventArgs e)
An error occurred loading the new service form.
