Unknown result for Mental Health or Sub Abuse Screening doesn't transfer to Annual Review

An error is generated when opening Annual Review tab if there is screening record for Mental Health or Substance Abuse that has unknown as the value for the screening. Using Yes or No does not generate the same problem. The error is: 
Source:  - Index was out of range. Must be non-negative and less than the size of the collection.Parameter name: index 
at Client.frmClientInfo.LoadAnnualReviewControls() 
at Client.frmClientInfo.cmbAnnualYear_SelectedIndexChanged(Object sender, EventArgs e) 
An error occurred while updating the annual review for the selected year.
